What GPU/PSU should I get?

I'm looking for a replacement GPU to replace the one that came with my PC because everything else is good on my PC except for the GPU. I want one that can play games such as Black Ops 2, Skyrim, etc. on high settings. However, I don't know too much about computer building and all that stuff. I have one PCI-E x16 slot, but my current GPU is using it. I know that my power supply is not adequate for a good video card. Any suggestions for my GPU/PSU? I would like a relatively low cost (I'm willing to spend $400 tops for both the PSU and GPU). My PC cost $550 so I don't know if it's even worth upgrading it. I do, however, know a MUCH better build for around $700.

Case - NZXT Source 220
Case Dimensions - 19.5” x 8.1” x 18.5”
Processor - AMD FX-4300 Zambezi (3.8GHz/4.0GHz Turbo, 4MB L2 Cache) Quad Core
Processor Socket - AM3+
Motherboard - AMD 760G
GPU - NVIDIA GeForce GT 630
Video memory - 2GB
PSU - 300 Watt ATX
RAM - 8GB

i recomended you should take the corsair TX750M for the psu and and for the psu you can take the sapphire R9 270x or R9 270 the cards will perform well with your cpu :)

Did you even read the question? I know that I have to upgrade the PSU. And I want to spend $400 at most for BOTH the video card and power supply.
