My ASRock Z77 Extreme 4 seems to be having issues

Hi, well like I've said in the thread I think my motherboard is dying. So I noticed at first that my PC had started to have graphical glitches, so I though it might be my graphics card, but it seems to be fine, still runs games fine has fine temps and all that as well. Then I turned my PC on a couple of weeks ago and it beeped around 5 times, which led me to find that it meant that my motherboard isn't detecting my card, however the PC still runs fine other than a graphical glitch when I boot the machine after it being off over night (basically only when it's a cold boot), the computer beeping and the Debug screen on the motherboard saying D6, which also relates to it not detecting devices.

So I've been searching for a while now, and a lot of the helpful things I've found have been on these forums, sadly what I've tried so far hasn't worked. Clearing the CMOS didn't do anything, trying the card in another slot didn't help, updating the graphics card drivers didn't work either. Now I'm getting to a point where I'd like to try and pin point the problem, but I can't properly test for the problem, I don't have another PC I can test the card in, nor do I have another card to test in the PC. Assuming you guys try to help out I will be able to answer any questions that can lead to me figuring out what the issue might be and if there is a fix or if I need to get something RMA'd.

System Specs:
i7 3770K Ivy Bridge Socket 1155 LGA
ASRock Z77 Extreme4 (Version P1.30)
8 GB RAM
Gigabyte Radeon HD 7970
Anything else I can get on request.

Sadly I'm not at the computer just now, I'm at work, I'll try to post my voltages when I get home, what's the best way to gather those for you?

Also I've tried switching pcie slots, suppose I've not checked for bent pins or blew air into the socket so I guess I could try that when I get home, cheers.

Oh yeah the all the components were bought in November 2012

Okay, so I've looked over the card and it seems to be fine, it powers on and outputs a display, which is what makes me think it's a motherboard issue. I just got home not too long ago. The voltages I have are on this screen shot here: http://i.imgur.com/5sZoliu.png

I'm just hoping there is a fix, rather than needing to RMA the board or the card.

EDIT: So a bit of an update, while I was keeping I eye on the thread, I bought Watch_Dogs, and I've been playing the past few hours, seems to run perfectly fine on my system. Checked the temps mid way through my session and they have been fine, hovering about 50 or below. I'm really not sure what's going on with this error to be perfectly honest haha.
