Perfmon Hard Drive Highest Active Time Spikes

Hello, first time here, and thanks in advance to any help (I am a computer noobie). I'm having some serious issues and I am not exactly sure of the cause. My laptop (Dell XPS) is freezing every few seconds. I can still move my mouse and anything audio keeps playing, but the video and application fall behind. So games become unplayable and stuff as simple as scrolling because jittery. I have found this though, my hard drive seems to have something wrong.

These spikes are constantly going and the hard drive light on my computer is always flashing. Other than that, I am clueless.

This problem has been going on for at least a year (3 year old laptop), and I have been trying to solve it forever. I have run Malwarebytes scans, chkdsk scans, CCleaner, coretemp, and some others. No problems shown.

Brother, from the above screenshots I would say that the load on your hard drive is perfectly fine.
There is no program that is doing a lot of reading/writing.
Maybe the problem is with the dying HDD.
I myself have the same problem and I am using my laptop with a desktop HDD (6 years old HDD but still faster than the dying one).
It really took my portability but still my laptop is as fast now.
You can also check by doing so or arranging for another laptop HDD from friends.
Btw are you sure that the cause is your HDD?
Try looking for other indicators like high CPU/MEMORY usage or very fast spinning fans.
