GTX 660 Crashing (screen goes black during gaming)

Recently I have been noticing that my screen goes black during gaming for no apparent reason. The blackout lasts 3-5 seconds but sometimes the game will freeze up and crash completely.

I'm currently playing two games where I notice this. Smite and Dark Souls 2 (running ENB mod).

Initially, my screen would go black for a few seconds and my fps would drop to under 30 after the picture comes back back (norm is 100+). Restarting the game would fix the fps issue. Occasionally, I would get a message saying that the video card drivers have stopped working.

My first thought was that the card is overheating so I downloaded MSI afterburner. However, my temperatures were fine (65 C is the hottest I've seen it go).

Then I decided to upgrade my drivers. The latest drivers fixed the fps drop issue. Now the screen would go black for a few seconds, but there would be no fps hit when the picture came back.

Last week Nvidia released new drivers that seemed to further alleviate the issue, but did not fix it completely. I've noticed the frequency of the crashes decrease (from about 5+ per hour to about 2), but they still do occur.

Smite used to work fine until they released a graphics optimization patch a few months ago that got rid of DX11 support. That's when the problems started in that game. However, I seem to be the only one experiencing the problem which leads me to believe it is probably caused on my end.

Do you guys think this is a hardware issue or a driver/optimization issue?

The video card is a Gigabyte GTX 660 @ 2GB.

The rest of my system is:
i5 2310 @ 2.9 Ghz
Corsair CX500 PSU
8 GB RAM

I don't have any other games right now that I'm playing for extended periods of time to test.

Unfortunately I don't have another one.

Is there any metric in Afterburner I can monitor to tell me if the PSU is an issue?
