I have two duel screen monitors connected to my IBM D90. I also have a HDMI connected to my flat screen TV. While the monitors running off of both DVI ports work fine, I can't get connected to my TV through the HDMI port. If I connect my laptop, which have a HDMI port, I can see what's on it just fine. I don't see why this doesn't work.

They share the same connection, you can only use one or the other with DVI or HDMI

So could I use a DVI splitter for my duel monitors and run my HDMI TV from the other DVI port on the video card?

Splitter only duplicate what is coming out of that port. It will not extend your desktop to both your monitors.
Do you not have Displayport ports to use?

I would have to add a second video card that I have, that has a Displayport. Would that work?
