I'm having a strange problem on my gtx 760. She simply stopped giving boot connected to motherboard primary slot (16x).
Bios freezes shortly after appear the gigabyte logo, without errors, and it is not possible to enter the bios.
When I connect the card on second PCI-E slot (running at 4x) it works perfectly. I even made some stress test and no error.

My Setup
CPU - FX-6300
RAM - 2x4 1333 mhz
MOBO - Gigabyte GA-970A-D3P (also tested on a M5A78L-M LX3 and the same problem occurs)
GPU - EVGA GTX 760 ACX 2 GB
PSU - Corsair TX-650

Try the card in a different computer. Sounds like your pcie 16x slot has died. Oops I see that you have tried that. In that case, check the VERY CAREFULLY the gold contacts on the card. Even with a good magnifying glass. you might find that there is a broken trace to one or some debris preventing contact. I once rebuilt a video card and re-lacquered the board and accidentally got lacquer on the contacts. It wouldn't work of course.
