GTX 760 recognized by computer but won't display

I recently built myself a new computer and am having some issues troubleshooting the setup.

Windows device manager recognizes that I have a GTX 760 plugged in, but it does not acknowledge I have a video card when I check dxdiag and will not display my desktop on my monitor using its hdmi or dvi ports. I have installed the most recent nvidia drivers, though in the past, lack of drivers has never stopped me from using the display ports on a graphics card.

If it helps, I also cannot get my hdmi or dvi ports to work on my motherboard, yet my vga port seems to work just fine. The following are my specs:

intel i7 4970
EVGA GTX 760 2GB
1 tb hdd
600 watt Corsair PSU
Asrock z87 Extreme4 (mobo)
2x4GB hyperx genesis RAM

Does anyone have any idea how I can solve this issue?

Try GPU-Z and see if it's detecting the 760.
When you see device manager recognizes the 760, does it report that it's functioning properly?
Outside chance but check in BIOS and see if integrated (on the CPU I believe its Intel HD 4600) needs to be disabled to use PCIE GPU (doubt this is it but can't hurt to check)
Try a different monitor. No HDMI or DVI from a GPU and Mobo that are not reporting errors points squarely at a monitor with issues displaying digital signal.
