PC Crashing While Gaming...

So I'm having some big issues while trying to play some of my games such as CoD Ghost and NBA 2K14. While playing these games or even starting them my computer will go to a weird screen. Sometimes grey or sometimes brownish with many vertical lines. This locks my computer up and makes it restart. Now, I'm by no means a computer whiz but to me this sounds like a video card problem however I have ran some tests and everything appears to be fine with the card. Same with the processor. Nothing is overheating. All the fans are working just fine. I'm at a dead end as to what could be the problem. This has happened before but lately, it's impossible to play anything without this issue.

I'm looking for any leads that might help me resolve this problem. below are my PC's specs as well as a link to a video where you can see exactly what is happening. Hopefully that helps in diagnosing the problem:

CPU: AMD PHENOM II X6 1045T THUBAN 2.7GHZ AM3 CPU
Motherboard: MSI 760GM-P23 (FX) AMD Series Socket AM3
Video Card: Visiontek Radeon HD 6770 1GB GDDR5 PCIe
RAM: Kingston 8GB 1600MHz 10-10-10 DDR3 HyperX Red (Single Stick)
Monitor: HP Pavilion 27xi

That's the other issue I forgot to mention. A week ago my PSU went out on me after around 2 years. Now, I had still experienced this problem before the PSU blew but not as bad as now. The old PSU was 680w. Don't remember the brand but it definitely wasn't a recognized one. The new PSU that I bought as a temp is a Dynex 520w. I know, not the best choice but I needed a new one asap. So that's my PSU saga. :)
