Recent Troubles With Lag

Hi guys. I have a EVGA GTX 660, and have had it for about six months now. Overall its a nice card. I am having some issues though.

When I boot up my computer, my graphics card idles at 0% usage, for both the GPU itself and the memory in it. The fan sits at about 35%, at about 49C. Whenever I am playing a game (edit: i should state that I also watch streams, and I also get 99% usage in them as well) (varies from League of Legends, Diablo 3, The Forest, Fallout New Vegas, and more) the usage goes up like it should. Now for some of these I have them on low everything and it sits at 99% usage and nearly 75-80C. Even on lowest graphics it seems like it struggles. I'll get random spikes of lag for varied times anywhere from a minute to ten minutes to me closing the game because I get annoyed. I'm quite positive a GTX660 should not be struggling on most of the games I play, especially on low settings.

Another thing that bugs me, is that even after I close out the game, my graphics card wont go back to idling. It will sit at 99% usage (by the way, during all of this, my GPU Meter gadget will always report a GPU clock of 0MHz, Memclock of 3005MHz along with Shader Clock, PCB Temp, and Memory Controller all being 0) and I have no idea why. Nothing will be running other than maybe some programs in the taskbar. I've used process explorer to help figure out what is causing it, but the only thing that comes up is "crcss.exe" or something of the like, which is a Windows file. Even then, in the processor explorer it only goes to about 30-40. I don't know why my GPU would be sitting at 99%.

Anyways, I suppose I should state what I'm trying to get at:
Whats wrong? Is my GPU messing up, my motherboard, is my processor lacking? I don't know whats going on. Any help will be greatly appreciated!

what was your graphics card before? And when you installed the new GTX 660 did you clean your system of the older drivers?

verify with another GPU monitor that the clock is indeed 0 Mhz, smells like a defect honestly

I believe I had a Radeon 6750 previously. Made by Diamond. And yes, I cleared my system. I reformatted actually because simply removing the old drivers was giving me a hassle, and I needed to clear some stuff out anyways.

According to MSI Afterburner, my clock is anywhere from 1045-1176. I only ran Fallout New Vegas, which is on ultra everything. Also, during the game my GPU% changed a lot, but even in Afterburner after I close the game it just stays at 99%. Temps are 74C, computer has only been on 30 minutes maybe. Also, it seems like my graphics card nearly never goes over 1GB used.

memory usage isn't that off, even during games like Skyrim and BF4 my gtx 670 stays at around 500MB-1GB memory usage.

Temps are frosty, it's really weird how it stays at 99% usage though, do you have a friend or a store nearby that can test the card out for you? I think it might be a prob with the card itself
