Fans spin, no output from HDMI (I don't have a DVI today to test, will get one tomorrow). When I switch to IGD and run out VGA to monitor, it shows up in the device manager and says it's drivers are up to date. I'll go back to BIOS and switch to PEG (only other option) and try with HDMI and black screen.

What are my next steps to test and try to get output signal?

Do you have the additional power cables plugged in?

Do you have the HDMI plugged into the graphics card?

Do you have an another HDMI cable that you can try?

Did you try using the integrated graphics to eliminate the graphics card?

Connect the HDMI to a TV to verify that the monitor isn't the issue.

Here are two checklist on "no video output".

To answer your questions: Yes, the two PCI-E cables are plugged into the card.

When I plug HDMI into the graphics card, the monitor doesn't display anything.

I know the HDMI is good, I took it from another set-up that was working.

I've gone to integrated graphics and out both VGA and HDMI and the set-up works fine.

Thank you for the checklists, I'll start working my way through them and seeing if it helps.
