A couple questions/suggestions.
1. Are you using the Nvidia framerate limiter? You shouldn't, it's been buggy and breaks vsync ever since the 320.46 drivers. If you don't know where to find the Nvidia framerate limiter, then don't worry about it since it's disabled by default.
2. Disable adaptive vsync in your Nvidia Control Panel and set it to Standard, if you have it enabled. Adaptive vsync disables vsync if the performance falls below 60, which is likely to happen sometimes in WoW no matter what, due to all the people. Therefore, standard vsync is more desirable than adaptive in MMOs.
3. If you haven't already, update your drivers to the latest version.
Thank you for response. I've tried every combination of vsync and triple buffering and still am getting it. Two things, is there a chance I received a faulty gpu? Also could it be because of a dying monitor?
There's a risk of a faulty GPU from the low performance, but it's not very likely. Typically those cause artifacts and graphics bugs in games before they would cause screen tearing.
No, it's not a dying monitor. Screen tearing isn't a hardware issue, so the monitor not accepting vsync isn't a result of failing hardware on the monitor either.

Yeah I've completely wiped the drivers and re installed twice but to no avail. I've read countless forums and tried everything they've suggested but for what ever reason my is still there. If this helps at all I also get screen tearing on the actual desktop not just in game.
...This sounds familiar, if it's happening on the desktop as well. Are you sure it's really screen tearing, or does it happen all over the screen instead of just in 1 or 2 tiny lines? Do you have PC speakers, or are they built into the monitor?
In game its without a doubt screen tearing and in random spots and sometimes only in one spot, very erratic. As for the desktop when i move a window around ill get little tears on the edges and really bad ghosting which sometimes causes bugs.
Well, it's not what I was thinking of. I had some poorly shielded electronics interfering with my monitor once, creating something similar to screen tearing. But that doesn't really sound like the issue here.
I'd suggest you remove your video card, use the integrated graphics of your i5, and test on your desktop to see if the ghosting/tearing is still there. If it's gone, we'll know it's a problem with the video card. If it's still there, it's something else and your video card will be fine.
