Possible artifacting GTX 660 SC

Lately, i have been having more and more issues with my GPU. First i had the card getting up to 89c causing it to completely lock the computer up, preventing me from using ctrl alt delete. I fixed the crashing issue by adjusting fan speeds for the GPU and cleaning out the case. But lately, no matter which Nvidia driver i have installed, i get weird alien like green elliptical flickering objects popping up all over my screen. Along with that, i get weird texture stretching and tiny dots flickering around the screen. When that happens, i hear the GPU fan ramp up to higher rpms making me think it is heat related. Games that i have seen this issue persisting in have been Fallout New Vegas, Fallout 3, Skyrim, and Left 4 Dead 2. I posted a video on YouTube showing what i described below...
I am not sure if my psu is causing this problem or if my GPU is just going bad.
On a side note: I got the 660 and the psu at the same time. I got both items in December of 2012.
And yes, i do play with v sync on in all my games.

GPU: EVGA Geforce GTX 660SC 2GB (reference card design)
CPU: Intel Core I7 2700k
RAM: 6GB
PSU: Antec High Current Gamer 750 watts

Have you OC your video card's memory at all?

If so, return to stock settings...if not, I would contact EVGA about RMA'ing your card.

My GPU runs at stock speeds. No overclocking in EVGA precision x.
