GTX 780 GPU malfunction after 70 degrees

Hi, im experiencing some issues related to my GPU( Palit GTX 780 Super Jetstream). After purchasing a 144hz monitor, i started having some crashes while gaming: "nvidia driver stopped working and recovered". After some tests. I have noticed that my card start malfunctioning when it reaches 70º, after this temperature thresshold it crashes and recovers, it doesn't matter if it is overclocked or not, it crashes anyway, same for all games.

Some time ago also noticed that the fan does not speed up the way it should, it gets very hot while the fan speeds up slowly, so i had to install thundermaster and set a custom fan speeding schedule. I need help, does anyone know what could be the problem?
I must say that im using latest drivers available(337.88) and that i have installed and cleaned them several times.

Asrock Z77 extreme4
6 GB corsair dominator 1600
PSU epsilon 900w

I must say that i have passed MSI Kombustor and was stable at 78º, no problems after 1 hour, so it only happens while gaming.
