GPU Crashing applications or blue screening at 65 Degrees

I have tried everything, from reinstalling windows, bios updates, driver roll backs and new drivers, and installing new drivers.

I have.
HIS 270X
I5 3570K
Patriot 16GB Ram 1600MHz
120GB Kingston v300 SSD
1TB Seagate 7200RPM HDD
Z68MA-D2H-B3

Overclocking?

Restore it to default.

apply arctic silver to video card keep case open when testing add case fan in front of case for air intake after getting things operational

I have not been overclocking, and im not comfortable doing that. Would you think its the cooler?

Download and run HWInfo - Sensors and post the results - usually 3 screenshots on TinyPic or similar.

That will tell us a lot - temps, voltages, clocks, fan speeds etc

149 Fahrenheit ? is kinda hot my 9800gt clocked to 814ram @600 core 1/5 th overclock on laptop runs at 56-58 Celsius max

Doesn't show Vcore or GPU, etc. But looks like your psu isn't working well.

That only shows the gpu - at idle. Need to show it all.
