Black Screen Crash Problems

I have 2 different problems since I overclocked my gtx 570 (But now I revert it back to default setting). First, my PC display turns black and crash when playing games such as Battlefield 4, Dota 2, etc. Second, DirectX error (directx function "getdeviceremovedreason" failed with DXGI_ERROR_DEVICE_RESET: Device reset due to a badly formed command) appears when playing the same games (Battlefield 4,Dota 2, etc)
I don't know what causing this problems.
I've tested my gpu using OCCT, Unigine Heaven Benchmark, 3DMark 11 without any problems.

My PC specs:
CPU: Intel Core i7-2700K
GPU: EVGA GTX 570 DS HD 1280Mb
Cooler: Corsair H80 (CPU) & NZXT Kraken X40 (GPU)
PSU: Raidmax RX850AE 850watt 80+ gold
Mobo: ASUS P8Z68 Deluxe
 
Solution
FIXED: I've just found the ways. Replacing my gpu watercooling (Kraken X40) with heatsink and connect the original gpu fans solve all screen crash (Black,White,Blue,Green,.... other color) problems.

As anyone who has a fair amount of experience OC anything will tell you, simply because it passes a benchmark does not mean its a stable OC. If your driver is crashing (and thats what it looks like is happening) you may have to dial your OC back more to get stable, driver crash also happens at high temps, check your temp history right after a crash, if its high crank the fan up more.

Assuming you havent damage the card, reverting to factory settings should fix this. Then just dial your OC back a bit until it is stable.

Thanks for your answer. My temp (Max Load): CPU: 67C, GPU: 50C, Mobo: 39C. I think the problem isn't temp. I've also try to increase my gpu voltage without increasing gpu clock but the result is same.
