Seeking a good HTPC single-slot GPU

I have an HTPC which is about five years old, and it's got a 500 watt PSU.

The problem is, I've had an HD 7570 single slot GPU for about two years now and it's dying.
I do not understand the problem with AMD GPUs, or if I'm just unlucky. I purchased a HD 6670 for another computer the same time and it died recently. (I replaced it with a GTX 750 - that thing is amazing!)

Anyway, I'm looking for a cheap GPU with displayport support that will let me decode 1080p videos (maybe slightly higher resolutions too) without choking, and shader support so I can use emulators in RetroArch with various shaders to enhance picture quality ;)

I will not be gaming in this machine. Any "gaming" will be limited to Miniclip and Radical Aces (3dvia GPU acceleration). Seriously, I will not be running real games on this rig besides emulators.

I would have bought another GTX 750, but they are all dual-slot (some are single bracket, but the fans protrude and my sound card is directly below on the only other PCI-e slot.) I just want a cheap and simple GPU, Nvidia highly preferred, which will let me do the things above. Bonus points if it's fairly recent (like late 2012/2013).

The problem with both of those cards is that the are not single slot. Even if they are single bracket, they have a heatsink which protrudes below. I have a limited clearance, because there is a sound card on the only pci-e x1 slot, which is below. I mentioned this in the question.
