I have been having terrible gameplay for some time now. I have geforce gtx 760, intel i5 2320, 2tb HD, 12 gb ram and have not been able to figure it out. I mainly play CS:GO so this setup should be perfect. Today I ran a Windows Experience Index and got a terrible rating on disk data transfer rate. What might be causing?

First of all, what's the brand and model of your hard drive? Not all mechanical drives are created equal.
The more platters a drive has, the longer load times may be while it switches them. A 2TB drive will almost certainly have atleast two.

Do you defrag frequently? I know for a fact that even one game, due to their large file sizes, can create a percentage of fragmentation on the disk. Windows may say it's set up to do this automatically, but the ah heck never does.

Did you set your drive to ACHI mode before installing Windows on to it? (From IDE). It really doesn't matter much for mechanical drives versus SSDs but I suppose it could be a factor.

Finally, your motherboard, it's probably very recent, but check what the SATA ports are running at (Current is SATA3, or aka SATA 6gb/s) and if you have them plugged in to the right ports and not the little extra third-party driver ones. If those are even a thing on your board.
