Do I need a new CPU cooler?

I recently built a computer, which has been working fine since I built it. However, lately it's begun to turn off randomly. This happens maybe once a week. Here is the CPUID report:http://imgur.com/ZTR7Wd8
I am currently using the cooler that came with my APU. I am not overclocking it. Do I need a new cooler, or is this problem caused by something else?

For a start - get a new power supply. Its putting out 15V instead of 12V!!!!!!!!!!!!

then post back temps etc.

Normally, I wouldn't think the cooler would have anything to do with your shutdowns. But if that temp is accurate, there is definitely something wrong with the cooling. That temp is at the throttle stage. Shut down comes next. It says the fan is at 100%. Is it in fact spinning?

You may want to check if the cooler has come loose. At least remove it, clean off the old TIM, and put new on. Then try the system and check temps.
