XFX R9 290 100% activity and gpu clock at desktop

Proof of 100% activity.) This issue is causing major game lag (Sniper 3 on medium settings and 20FPS.) I have no idea what the issue could even be with the card so any help would be appreciated. If needed more information just ask. My motherboard also came up with code 0040 displayed. (normally 00A0) I have no idea how to find the meaning of the error and have tried.

From your attached image, I'd guess that the 150mhz memory clock could be part of the problem causing your low FPS. Since the GPU clock is still at (near) top speed, I don't think the software is putting the GPU into a low power/idle state, otherwise both the core and memory rates would be low, not just the memory.

Have you manually set any of the power target, temp target or changed the core / memory rates?
Have you tried fully uninstalling and reinstalling the GPU drivers and catalyst control center?
I ask about the drivers and control center since the software is reporting target values of 0% across the board while also power consumption is at '0%' as well.
