Unstable Gaming Problem

Hi all,
Thank you very much for checking this post.
I assembled my mini-itx pc yesterday. Win7 Home Premium SP1 installed. Used Geforece 377.88.
My problem is: The systems runs totally OKEY when not playing games, i.e. online surfing, watching video, etc.
However when I playing games, the monitor will occasionally black out for 1~2 sec, and then 1) the game might crush (like Dota2), or 2) it will get frozen for couple more sec and then return normal.
I didn't mark the time however its more like once each 1~3 hours. Can't predict when it will happen---for this morning till afternoon nothing happened, and from later afternoon till night it happened twice. I'm not a hard gamer but I intensively tested today.
I'v been tested on following games:

Dota2 (totally about 4 hours played, it happened 3 times and the game just crushed. However the game is still running background since all music/sound is still good;)

Diablo3 (totally about 4 hours played, blacked out 3 times. the game crushed completely once (just jumped back to desktop) and for another 2 times it blacked out for like 4~5 secs totally and then returned normal)

StarCraft (totally about 4 hours played, blacked out 3 times and returned normal after all)
Simcity5 (totally about 3 hours palyed, blacked out once and returned normal)

All games running under either high or extreme/ultra setting. Games runs fine when those annoying black ours not present. Before the blackout happened, the graphic will be kind laggy (but only for a very short period).

Also all games were installed on the external HDD----i don't think it will be a problem...but correct me if i'm wrong.

System temp always monitored. For most cases, CPU remains around 50; GPU remains from 40~67; MB around 50.

A clean re-install seems doesn't work. I did it once.
Initially i thought it was the problem of either graphic card or memory. So I did some fatigue test/stability test on both of them.

Graphic: Both 3DMark and Furmark. Nothing happened during the test. (no blackouts, etc); For Furmark, TDP around 100% for 15 mins, highest temp was 68.
Ram: Memtest86. No error after one pass(i read on other threads and it was said 99% problem will happen during first pass)
Overall: Aida64 test on CPU, Ram and graphic.

I'm so confused. Any thoughts?

CPU: I3-3210 IVY Bridge;
M/B: ASRock H61MV-itx
Ram: Corsair 4G
HDD: Sandisk 64GB SSD+2T external HDD
Graphic: MSI GTX750Ti 2G
PSU: Coolermaster 350W
Chassis: Coolermaster Elite 130.

CPU temp min 28, max 38
GPU min 26 max 28, fan 34%, voltage 0.962.
I'll run a game text and see.
