Graphics Card Crashing (I think). Fans Spin Like crazy

I have a self-made computer and it's about 3 years old. I usually play games like Civilization V and Skyrim. My brother also plays NBA 2K14. The computer barely crashes nor lags. I always run at the highest graphics settings (unless its Battlefield 4) with no problem. For about 3 months now in the middle of playing games my fans start spinning real fast and sooner or later if I don't turn down my graphics the monitor turns off. Also, my fans go really really really fast and I have to force quit the computer. The reason I think it's related to graphics is because of 2 reasons.

1, On some games if I play at low graphics my computer doesn't crash or takes longer before it crashes.
2. When I click on the little flag you see at the system tray (bottom right of your screen for windows 7) it says there's a problem with NVIDIA Geforce.

Also here are my computer specs:
Windows 7 Ultimate
8 GB RAM
NVIDIA Geforce GTX 580
64-bit
Intel(R) Core(TM) i7-2600K CPU @ 3.30GHz

It seems as if your GPU or CPU are overheating. Can you download CoreTemp and monitor the CPU temperature during gameplay? Also, download MSI Afterburner, it has a useful GPU temp monitor built in with the overclocking software. Open up this program (don't apply any overclocking) during gameplay, there's a temp monitor on the right hand side of the window. When the fan broke on my GPU, I had this same thing happen to me (the fans going loud and the screen going blank).

I downloaded MSI after burner and saw just on the web it was at 58 C. Then I loaded up Skyrim and when it was near crashing I checked my temp and it was about 91 C

(Maybe if i waited a bit longer I could've got a higher temp)
Also when it was 91 C my fans were going very fast
I checked my fan speed while gaming and it was about double the speed when we browsing

Sorry for the late response. I did some research and found out that the highest safe operating temp for your GPU is 97C, so it doesn't look like your GPU is the problem. Did you check your CPU temp while gaming?
