HDD problem, slow response, 100% usage and more...

Hey, so basically a few days ago my computer started "slowing down", it got worse the days after. The computer needs about 15-20min to boot into windows, and then i need to wait on a black screen with a cursor for the desktop to appear (or relaunch explorer.exe). I looked on the internet and saw that its probably a hdd problem.i have a SeaGate Barraccuda 320GB hdd btw... I scanned and cleaned my computer of unwanted stuff with malwarebytes and also used ccleaner, I even opened the computer and cleaned it from the inside, didnt work so I formatted one partition and then splitted it into 2 partitions, 1 with about 40gb reserved for installing a new windows (8.1 pro x64). So I installed it and thought it could be better but no, now im 90% sure that its the hdd causing the lag and freezes. In the task menager it says that disk usage is constantly 100%. I disabled some services that people said will help to lower the usage but again, didnt work. Today when i woke up, i powered the computer on and it said "a disk read error occurred, press ctrl+alt+delete to blablabla" so i did and it restarted, and the boot screen couldnt recognize/or it needed a long time to recognize the hdd. I opened the computer and switched cables and SATA ports but again, it didnt fking work. In the bios, the hdd is recognized and i tried running short smart tests, it goes to 96% and says failed. Now im writing this on my phone while the windows 8 logo finally appeared, i guess windows will boot in about 30 min max. So yeah, the problem is the hdd, its not dead yet but its ultra unresponsive, I mean it responds but ultra slow. If anyone can help me, that would be awesome ! The hdd is out of warranty and i srsly need the data from it (20gb of important data only tho, mostly youtube stuff and photoshop work etc). I live in Croatia and i dobt think there are any data recovery "people" out here xD
