What is causing this? (short video included to show the problem)

This happens when I connect my tv to the hdmi port on the 780

Specs:
EVGA GTX 780 3gig
Asus Z97-A mobo
i5-4670k
Win 8.1 64 bit

What I've tried:
Different HDMI cable
Different TV (to make sure the problem isn't the tv)
Different drivers for the video card
No drivers at all for the video card
Unplugging card and using just the mobo (there is no issue when I use just the mobo HDMI)
Adjusting the audio quality via control panel to different hertz.
Updating firmware of my tv
Different BIOS drivers
The GPU in a different system (it worked fine)
Lastly, there is no issue if I use a 3.5 mm headphone, or a USB headset.

HELP

**EDIT: Major update**

Using a prgram called DPC latency checker, I see that latency might be the problem. See video here:

Theres something wrong with the HDMI port on the GTX 780, check to see if there is not dust or dirt in the port

Just spoke to an evga tech support, he says it may be the PSU. Possible? It's a Corsair CS650M...

Yea that could be the problem, that specific PSU isnt all that reliable

I doubt that, though I'm not ruling it out.

However, I think I have found the problem,
but not the ultimate cause. I used a program called latency checker and I see that latency spikes whenever an audio file is played: please watch
