Can't find bootable OC profile (PZ77-V + i5-3570k)

I've tried a few profiles, and the result is the same -- the comp starts but doesn't get to boot. Just hangs, no vga, no mouse, keyboard etc. and I have to reset via CMOS to revert to bootable state.

I've tried:

4.2ghz @ 1.25v
4.0ghz @ 1.23v

I didn't want to go much lower because I've been led to believe this CPU is more than capable of such an overclock, so maybe it is something else? How would I debug this? Keep reducing speed/voltage?

are these settings preset or manual settings
anyhow manually set voltage to 1.25v fixed voltage turn off c-states and turbo boost and then up the multiplier until you crash back off one multiplier then run prime for an hour to stability test your system
