Usb drive needs to be formatted but shows that windows was unable to complete the format

I have 64gb usb drive and i need to format it but every time i do it. it shows that "windows was unable to complete the format". And I have tried to run it to command prompt and this is what i got.

"Type of system is exFAT.
Windows is verifying files & folders...
The uppercase file content is incorrect.
Corruption was found while examining the volume bitmap.
Corruption was found while examining files and directories.
An error occurred while examining the volume bitmap."

What could be the problem? Please help...Thank you!

Have you checked this drive with a diagnostic tool or the Windows CHDSK? Is it healthy or repairable? If you also can not make sure, you may ask some professional drive agencies for help.

Yeah, i did that chkdsk and it always appear to have an error while examining it.

it will be asking you to format the disk and if you click that format a tab will appear that windows was unable to complete the format.
