Is my HDD starting to die?

Hello everyone,

I would like to start off by stating that my computer is from 2011, I have had it for 3 years now. It was a pre-built computer, however over time, I have replaced every single component, except for my HDD and my motherboard. In the recent month, I've noticed that my computer tends to break once every 4-6 weeks, mainly involving a BSOD which requires me to re-install Windows. On many occasions, there is also corrupt data, whena disk check runs up on startup. In general, my computer is very clean, apart from all the necessary files and drivers for my system, I only have a couple of enchanting programs and a couple of games (I use this computer mainly for gaming.)

Anyway, I recently got another BSOD, the one that appears right after the Windows 7 logo and restarts your PC, so I re-installed Windows. It wouldn't make a huge difference for me, although I'm starting my holidays soon and I don't want to experience any computer problems then. My question is, is my HDD starting to fail and if so, should I get a new one? I've used several programs to check the life of my HDD, although most of them said it's 100% functional. I highly doubt anything else could be causing me problems, although if anyone has any other ideas, I'd be happy to hear them.
