My monitor keeps disconnecting intermittently a few minutes after I start the computer and login to Windows. I have tried:

- a different cable
- a different monitor
- change the power settings to 'Never' switch off display

but the problem remains. Only the monitor gets blank and shows the words "No Signal", but everything else is still working, including the sound. Is this a graphics card problem, software or some other issue?

Update: The computer seems to work in "Safe Mode with Networking". So, maybe I can eliminate hardware problem? Probably the graphics card driver or other software problem?

Solution
The only way to be sure is to try with a different graphics card. But you can try using a different output (VGA for instance, if the monitor supports it). If you have integrated graphics on your mobo or the CPU, it might come in handy in this situation for testing.

BTW, please list your entire specs, including the exact model of your PSU.

Thanks for the help. It seems to work in "Safe Mode with Networking". Does this mean it is a software problem? I have tried updating the graphics card driver but it seems to be unable to successfully do so. How do I safely uninstall and re-install the graphics driver?

It is indeed a driver issue, could you confirm what graphics card you have and I will provide you with instructions of how to completely un-install your drivers (not just from add and remove) and we will work from there to get it working again.
