4gb ddr2 ram 800mhz
radeon r7 250x 1gb ddr5

can I play
assassins creed unity
far cry 4
gta 5
sims 4
fifa 15
battlefield hardline
cod advanced warfare
will I get 30 fps minimum on low settings what about medium if well optimized

Far cry 4 and gta 5 will push your e8400 to the limit, and battlefield multiplayer will push it even harder

will the game I mentioned work on my pc with 30 fps on low
ac 4 requires quad core but I am able to play that on decent fps

I think it will be close with FC4 / GTA5 but should be playable, but battlefield multiplayer I think you will be going under 20fps a lot.

If you can overclock at all then do it, the e8400 OCs like a champ :D
