1. Most people don't, at least when it comes to gamers.

Not true - especially when a lot of gamers are actually embracing the small form factor builds. Give it a couple of years and you'll start seeing companies market cases and motherboards exclusively for mATX and mITX systems, EVGA already got started with their Hadron Air and Z97 Stinger series. Bitfenix also capitalized on SFF builds a few years ago when they started heavily marketing the Prodigy and now the Prodigy M.

Some people do use mATX cases, but gamers who build their PCs usually use ATX mid towers, at least all of the gamers I know do. From what I've seen most people who ask for build advice here on Tom's also use mid towers so that seems to correlate with my personal experience. Of course it depends on the definition of 'most', I meant most as in 'over 50%'. Prebuilt PCs and HTPCs are a completely different story altogether, although I should have clarified that I meant self-built PCs.

Well I agree there, but I think the trend is going to be moving away from ridiculously huge towers and going toward smaller ones. I also think that we'll see the trend of multiple optical drives go away. I'm not saying there's not going to be a market for huge towers - you'll still need one for a large graphics or liquid cooling setup, but most people will be moving away from large towers - and cases like the Bitfenix Prodigy and EVGA Hadron will become the norm.
