Sapphire radeon HD6850 diagnosis of failure (video of problem inside)

Hello

Symptoms

I have had the HD6850 for 3 years and for the last 3 months or so I have experienced black screening during gaming. Recently, searching for a solution, I removed my graphics card, used an air duster to remove all the dust, removed thermal paste and then applied a new layer (after carefully watching videos and using appropriate methods). Since I have done that my graphics card has been seriously failing when I try and run 3D applications, mainly games (apart from hearthstone for some reason) I do not know how to describe what happens to my screen so here is a link to a video of what's going on demonstrated using OCCT. If a higher resolution video would be useful I will happily provide one. A diagnosis/reason as to why it's going wrong would be ideal if that's possible.

OS: Windows 7 64 bit

Motherboard: Asus M5A97 PRO Motherboard
CPU: AMD HDZ965FBGMBOX Phenom II X4 965
PSU: Corsair CMPSU-600CXV2UK Builder Series 600W Power Supply
RAM: Corsair CMZ8GX3M2A1600C9 Vengeance 8GB
Monitor in case it's useful: BenQ GL2450HM 1920x1080 monitor

My setup is not new, most of it I have had for 2-3 years. Let me know of any additional information I can provide you with, I will get back to you asap. If my graphics card is beyond saving recommendations of a replacement would be much appreciated.

Looks like one or more of the chips isn't getting cooled enough, check the loaded GPUTS1 MemIO temperatures-145C max!
I'm guessing either:
During reassembly you missed out or damaged a small memory/ chip heatsink, although I'm not even sure this card uses them.
The fan is running slower than its reported speed, causing the overheat, try setting its speed manually through the Catalyst Control Centre to 100% and retesting.

it peaked to 113.5 degrees celsius while in 1080p in fullscreen and was then 96 degrees celsius in 1280x720 windowed mode. This test was done with the manual fan control set to 100. Do you think that opening up the card again, cleaning the area and re-applying the thermal paste would be a good idea?
