It may be the PSU is not being able to keep the +12V up. Or it could mean the card is defective.
Is there a program to check psu usage, should i revert my i5 cpu back to stock instead of overclocking
It will depend on what the system consists of and how high the OC is. And the make/model of the PSU.
The PSU usage is simply watts. There is no pgm that I am aware of that will display load side watts accurately.
