GTA4 Lag Spikes

Now, before I ask. I need to make a couple of points.

Firstly, I know that many, many people have asked this exact question before. I have been through quite literally hundreds of threads on a ton of different forums, but nothing on any of these forums has fixed my issue.
Secondly, I know that GTA 4 is an ancient game in the scheme of things, and that it was probably the most horrific port ever seen on PC. But, I still need to fix this.

Okay, so first of all I shall describe my exact problem.

When playing GTA 4, I get a pretty high frame rate on maxed settings (between 25-60, averages at around 42.) But, every so often, a lot of the time when rendering something the frame rate drops for a little under a second to about 15fps, then jumps back up to around 40 again. I can sometimes play smoothly for about a minute & a half, then it will do the same. Before anybody says it's because I can't run it on max, you may be right. But, I get the exact same thing happening when everything is set the absolute minimum settings. (Including textures & resolution.) Plus, I don't get this glitch in any other game.

Now, the important part. My computers spec:

My main drive with my OS & Games on is a 1TB Western Digital Green HDD, I also have a 1.5TB WD drive for backups/downloads etc..

RAM: 8GB DDR3 Kingston 1333MHz
CPU: AMD Vishera FX6300 3.5GHz
GPU: AMD Radeon R9 280 (not the 280X, it's the step between the 270X and the 280X. Just a rebranded 7950.)

Now, correct me if I'm wrong, but that system should probably be able to max it out. (excluding draw distance) at a pretty decent frame rate. But, that's not the point. Because it should definitely get a solid FPS at minimum settings...

Things I have tried:
Since it's a 3GB card, I had to do the -nomemrestrict thing, I have also disabled event logging, disabled any sort of CPU/RAM hog in the background on my PC, played around continuously with the settings in CCC, overclocked/underclocked the CPU & GPU, etc..

Before anyone says there's a bottleneck, I've checked that too..

The GPU tends to sit between 55-80% load, while the CPU sits at around 40-80%, while around 3.5GB of RAM is being used. I've tested that extensively and never once saw any sign of some sort of bottleneck.

Now, onto the annoying part.
My old system was basically the same except it had a bulldozer generation FX4100 quad core CPU & an ASUS GTX 550ti 1GB, this could run it on high with a sold FPS and no frame drops what-so-ever. One other thing to add, I get the same problem in Windows 7 64bit & Windows 8 64bit.

Sorry this dragged on a little bit, but I wanted to say everything I could think of to save having to answer lots of questions about my build/os, etc..

Also, forgot to add this happens in 1.0.4.0, 1.0.6.0 & the latest 1.0.7.0.

While GTA IV is an extremely terrible port, it's kinda odd considering you were getting good frames with your old setup but not the new one. What patch were you using on the old one? Plus, is it EFLC or just the regular bowling simulator GTA IV? I'd recommend trying patch 1.0.4.0 and 1.0.7.0. As far as I know, 1.0.4.0 is more mod-friendly while 1.0.7.0 is more optimized. For me, 1.0.4.0 always worked with better performance. Maybe try that?
