Graphics Card Crashing Problem

Hello smarter people than me:

Ok so I keep having problems with graphics cards. I have the GeForce GTX 660 I had problems with it crashing where it would go black screen then comeback and say

"Display driver stopped responding and has recovered
-Display driver NVIDIA Windows Kernel Mde Driver, Version 335.23 stopped responding and has successfully recovered."

I had alot of problems with that and the guys at NVIDIA had me try everything and the eventually EVGA let me return the card for a new one. "GeForce GTX 660" That was on May 20th that I returned that card and the new one worked all up until July 18th...I got a blackscreen and my pc froze. then I started getting the same driver stopped responding and has recovered but this time said: http://gyazo.com/028740cc6a6d0d14fdbe81d55bd44470

What do I do I do alot of photoshop work and video work(YouTube) and also play Minecraft PC. Do I need to have them replace my card again or will I need to get them to get me a different card like different version?

It's getting to the point to where I think its the style of card doesnt work with my motherboard which is: ASRock Z77 Extreme 4

I have a built PC btw:
Intel Core i5 3.4GHz
Windows 7 64 Bit
16 GB
ASRock Z77 Extreme 4

From the Most Recent shutdown Windows showed this was the problem so if you get anything out of this great!

I am the only one that I know that has a built pc so no it's not possible. I'm not the smartest guy when it comes to pc's what do you mean by "Cheap secondary caps"

Ok so what do you recommend I should do.

Oh btw the first time I tried to turn it on today it wouldn't turn on so I had to use the power button on the motherboard after unplugging and turning off the power switch.

Nope not the same parts, not even the same manufacture. the CX400 was made by Seasonic, the Cx500 is made by CWT(Channel Well). The CX400 was the only good CX(I say was because its discontunued) but it was a rare one. The rest of the CX series did initially review well, as they are usually good out of the gate, but the cheaper secondary capacitors start to degrade over time and heat will speed up the process. When they first came out I bought 5 of them based on reviews and the price was good. After the first year 4 of them had failed. They were being used on systems that would not tax them or even use anything close to their rated power but they start showing system instability and/or started to cause the system to shutdown.

CWT is hit and miss they make some decent units(like some of corsairs TX units), but they have a few bad ones aswell. I would say the best PSU manufactures are Seasonic, Superflower and Flextronics.
