Black screen crashes with sound looping

Okay so I've been pursuing this issue for a while now with no real fix. And so now I'm coming to you guys for help.

Basically, when playing either CS:GO or League of Legends (Only two games I've experienced the crashes in so far [I think I might of had one in GRID:Autosport]) the monitor will flash then go into standby (Steady pulsing as if the computer was off). When the crash occurs in CS:GO, I will hear a sound loop. When I crash in League of Legends, it's as if I disconnect, all the ambient sounds are there but nothing else. However external programs like Skype will continue to run fine (I can still communicate with whomever I'm contacting) but the monitor is just.. off?

Specs:
CPU: Intel i5-4690k
GPU: Gigabyte Windforce R9 290
MOBO: Asus Maximus VII Hero
RAM: Corsair Dominator Platinum 1866MHz 8GB
PSU: EVGA Supernova 750W G2
HDDS: Samsung 840 Evo 250gb + 1TB WD Black

Originally I thought the issue was the RAM (after much researching), so I went ahead and downloaded memtest86+. On the first pass I had an error in test 5. I only completed one pass as I had run out of time and still needed to use the computer. Since there was an error I will be sending back the kit for RMA shortly.

I then swapped out the RAM for my brother's 16GB HyperX Fury 1866, which I ran one pass on memtest86+ and didn't turn up any errors.

However, I'm still getting these black screen crashes.

I'm now thinking it could be the graphics card (hoping it's not the motherboard) that is causing the crashes. Is there anyway to be certain? This is the most inconvenient part to replace since it is the only part apart from my SSD that has been shipped in from interstate.

Can you please help me identify the cause of the crashes? This is my very first build and for there to be this problem is very disheartening for future builds.

You seem to have tried lot already, so this is a bit of a longshot:

A while ago, I got similar crashes as you in DOTA2. It turned out to be some RAM that was running faster than it's spec. So make sure in the BIOS you check to see if your RAM speed is correct.

What you could do to see if it's your vidcard, is take it out and run on your motherboards video output. The performance will be terrible, but I guess you could run LOL and test to see if it crashes.

I had GPU-Z running in the background logging my GPU information whilst I was playing CS:GO. It did crash whilst I was playing, however I'm not sure whether or not the logger is still active whilst I have the black screen or not.

Is it an anomaly to have the memory clock speed randomly drop to 150Mhz? And what about the load jumping from 90->0 all the time?
