GPU Fan only starts in Afterburner

First here is my rig:
Motherboard: Gigabyte GA-Z97X-UD5H-BK
Memory: 2x4GB Patriot Viper Xtreme
CPU: i5-4670k @ 3.5 Ghz
Graphics Card: PNY Geforce GTX 660
PSU: Corsair 600W

Ever since I built my computer, the fan on the GPU would only start if I manually started it in Afterburner. Even if i was playing a game at max settings the fan would not start. Something interesting is that I am getting random temperature spikes of around 50C. I know that this temperature is not very hot at all, but when this happens my screen and audio both seize up.
At first I thought this was an issue with my drivers, and I proceeded to update them (currently on 337.88). Even after doing this I continued to have my problem.

Update: I have set a custom profile on afterburner, and with that the fan works fine, the issue now is whenever the card reaches right above 50C (as in exactly 51C) my screen freezes, and the card cools down to around 40C and works fine. After a couple of these freezes my computer completely stops working and becomes unresponsive.
 
Solution
Well it's never good to have freezing. If it's that repeatable (exactly 51C), then it's likely your GPU is failing. 51C isn't that hot, which means this GPU has a weakness (or the solder holding it to the PCB) that is sensitive to temperature.

If your card is under warranty, I'd set up an RMA with PNY. Tell them the issues you have (fan, and temp issue), this should get you a new card.

One thing you may want to try and hasn't been suggested yet, can you try this card in another system. If you can, see if the fan behaves the same. Also check to see if it crashes the computer at the same temperature.

Can you maybe just set a fan profile in afterburner? That will make your card's fans spin only when needed. Not sure why it never spins normally, very odd.

I would start with that and see if the temperature spikes remain.
 
Seems to me that something isn't quite right in your graphics cards BIOS. The fan should start as soon as you power on your system. Once control has been handed over to the OS / driver / software utility, the fan speed may change, but would likely stay going.

There is one caveat, if the card is marketed as a silent card, then this could be normal behavior. Something like ASUS's new STRIX which keeps the fan off until it hits a predetermined temperature.

That said, if you are playing a game at max settings and the fan doesn't come on, I would assume it's past what most people would consider safe for your card. Can you recall what it was reaching under these circumstances?

I would contact PNY, maybe they'll have you flash your card. Sometimes early releases have bugs in the BIOS of the card and newer releases fix these problems. It's obviously not a problem with the fan control circuitry or fan monitoring circuitry as you can get the fan to operate in AB.

I do not have any other applications controlling fan to my knowledge, and even with the afterburner profile, I still experience these freezes.
