Asus Maximus IV Usb Hotplug Error

I've browsed around forums and google around but I can't seem to find any help for my situation. I built a rig a few years ago and it's been giving me problems, but only when I don't power it up for extended periods of time (over a week).

Usually it will either not turn on our reboot endlessly, and so far I've been able to fix it by resetting the CMOS (with the button on my motherboard) or messing with the usb ports.

Now when I power it up neither monitor gets a video signal and I get the debug code b4 (the handbook describes this as "usb hotplug error), displayed on the motherboard. Ive tried resetting the CMOS, unplugging certain or all usb devices, rebooting numerous times, etc, but I still cant get it to boot properly.

I have an Asus Maximus IV Extreme-z motherboard, an nvidia gtx 570, 2x4 gigs Corsair Vengeance ram, and a CoolerMaster HAF 932 case.

Literally the only time I have problems with this rig have been when certain rear usb ports randomly stop working for a bit. Also the front ports on my I/O panel are broken, so I've disconnected them from the motherboard.

Any suggestions would be welcome. Thanks everyone
