I've searched for hours for this same question and i've found nothing that has worked for me, so maybe it's a faulty card. Here we go.

The graphics card and motherboard I got from amazon warehouse deals. The mobo seems to be working fine but the 760 isn't recognized in either PCI 3 slot. The 8 and 6 slot power things are plugged in so the fans spin. I've updated mobo drivers and turned off integrated graphics in the BIOS. Tried using nvidia and gigabyte graphics drivers but the card never shows up on the Device manager, only the Intel HD 4600 that come with the processor.

Specs:
750 watt Rosewill PSU 80 Gold
i5 4690k 3.5ghz
Gigabyte GTX 760 OC revision 2
MSI Z87M gaming motherboard (the one with the dragon)
8gigs Team DDR3 1600mhz RAM
Samsung SSD 128gb
1TB 7600 RPM Maxtor

Are you plugging in the monitor cables into the actual GPU itself rather than on the motherboard? Just from your description it is functioning properly. Also if you've successfully disabled your integrated GPU then how did you use your computer at all without any graphical interface device if your card wasn't functioning?

This isn't the first time i've booted it up, like I said I have the mobo drivers up to date. The i5 has graphics drivers that work, though are limited. I've tried plugging the cables into all places. The BIOS doesn't even see the card when it's seated (and yes I've reseated it a lot and in different pci slots)

I believe it's just a dead card. Or the mobo PCI slots don't work.

I suppose maybe the contact points were damaged during shipment so that's why it can have power fed to it and still have the fans running but the card itself not being recognized. If the slot is feeding power to it then it is probably functional. I'd go to your GPU vendor's website and see what they have to say and if you can RMA it.
