Need Help Voltage Troubleshooting

So for the past few days I have been getting warnings from my Asus Sabertooth 990FX motherboard about voltage spikes or dips. These changes occur at random, my best guess is they occur when my system is under load. In the past 24 hours I have noted the following changes:
12V dropped to 1V
3.3V jumped to 4.416V
Vcore jumped to 3.060. (normally 1.260V)
3.3V jumped to 6.1V
5V dropped to 0. 81V.

When I collected these results I was just downloading on Steam, and typing.

So my questions is what is causing this to happen is the PSU going? Motherboard? or is the software that came with the motherboard just faulty?

Something is making up voltage readings. If your 12V rail dropped below about 11.4V the system would cut out, if the 3.3V rail made it to 4.4V you would blow up your RAM, HDD, and some chips on the motherboard.

What software are you using to monitor the voltage on the system?

I am using AI Suite II. That is the software that came with my motherboard when I got it.
