Monitor went crazy

Hi all,

I'm hoping you can help me with my problem. Let me being with a brief background.

My computer for the past 3 weeks or so has on occasion been going to a blue screen and restarting. I sadly have no clue what the blue screen is about because it happens so fast. I want to say that I saw something.atx or something of that sort on the screen.

My computer will also randomly freeze up for 20-40 seconds before returning to normal.

Another random thing is if my computer goes into sleep mode, when I turn it back on, the USB 3 mouse I have isn't recognized and I have to unplug it and replug it in.

The final issue that led me to post here is that I was in a round of Hearthstone when all of a sudden my game froze. Then the monitors displayed static looking lines (it reminded me of having a TV on while being on a channel that didn't have anything).

I'm at a loss of what the issue is. I've reformatted my drive at least twice, and done fresh installs of everything. I've also reinstalled my GPU driver.

I went ahead and downloaded whocrashed. It told me it was my atikmdag.sys module. I went ahead and reinstalled my GPU driver and so far so good. I'll give an update soon but I think that may have been the problem for some reason.
