The most efficient upgrade -- assuming your PSU isn't ancient and a complete hunk o' junk -- is a 750ti, which doesn't use much power and plays most new games very respectably. Though you'll still be held back to a degree by what is a very old CPU. 7-8 years without upgrades will usually leave a PC very difficult to upgrade incrementally and get full value from parts.
