Graphics card stuck overclocked

i overclocked my xfx radeon hd 6670 using msi afterburner. when i pla games its fine for a few minutes and then it freezes and makes my pc have brown lines down my screen. this only happens when i play games and the only way to fix it is if i reset my pc.

rest of pc:
msi 970a-g43
adata premier pro 1333MHz 4gb (oc to 1600mhz)
400 watt coolmax
amd fx 6100 3.3GHz

i already did this but everytime i boot my pc up, it says that the clock speeds are at 940 and 1260 even tho i have them set to 900 and 1000. i had msi afterburner on my pc before and i set the speeds to factory settings and i uninstalled it. i also have the setting in catalyst control center set to 900 1000 but it still sets them to 94 and 1260.
