Check temperatures on your GPU. If it's overheating, that can happen.

Screen looks like a dead card. Try another card.

Check Control Panle, Device Manager to see if your card is working properly and whether the drivers need updating.

Do a checkdisk with repair in case you've had a drive hiccup.

Run a virus scan on your drive eg Avast and Malwarebytes

Read through this, although it is from nvidia it still is relevant to you. Lots of things which could cause this. The screen you posted is weird though, really seems like a dead card. Is your PSU working correctly?

Scroll down a bit to where they list the causes of this problem. Go through that list and check ALL of the listed items.

I did not have drive hiccup.
I checked control panel it is working properly
I scanned with malwarebytes nothing
I replaced the card twice so I don't think the card is dead

I really love the users on this website. Most of them do not know what they are talking about and post some basic trouble tech tips/bullshit.

Once again, are you sure your PSU is okay? If you've got 2 cards die already and this is the third one it might be your PSU causing this. Not saying that this is the answer to your question, but it could be a cause. Also, check this link as it describes your error perfectly and contains better info than what you will get here.
