Both PCIe power leads connected to the card? Monitor cable connected to the card?

Try re-seating the card (Or putting it in the other PCI-e slot) and power cables to it. Then I'd go for a CMOS clear.

Happens and glad it was simple but to answer your question it happens always even with new hardware that people get faulty hardware out of the box. It is a very small percentage of the total production but can happen to anyone regardless of manufacturer or quality.
