OCZ SSD Issues

Of late, when I start or restart my computer, there is lots of hanging time in the BIOS and the computer takes a hell of a lot longer to start than it should or used to. When the computer is in sleep mode, it also takes almost 5 minutes to return to regular functionality. I have connected, disconnected, and reconnected it in every possible way and this issue remains. There are a couple of strange issues however.

1) The OS is absolutely installed on the SSD.
2) When my other regular hard drives are unplugged, I get a "disk read error". When the others are installed, it starts up normally, (although slow as can be).
3) When the computer is on and I'm using it, it works perfectly except for chrome periodically freezing. Games work fine, however.
4) The SSD IS almost utilized completely. I have about 7 GB of space left on it. From what I've heard, that might be an issue, but I don't think it would cause this kind of problem.

Is my SSD failing?

I have an OCZ Agility 3 and two other regular hard drives in my computer. I built it about 4 years ago and added the SSD 3 years ago. Gigabyte UD3 Motherboard, 8 GB RAM, AMD 965 Black Edition 3.4 GHz quad core, Radeon 5770 HD 1 GB.

Thank you so much!

I did a long time ago, now is the only time there's been an issue. How do I check my other hard drives for that? I actually see it's not showing one of my other hard drives as connected right now.
