Xbox 360 Controller for Windows showing 2 controllers in game

My xbox controller for windows has started showing up in game as 2 controllers (when I choose sides) for FIFA 14. I've tried re-installing the software, and re-calibrating. I don't have an xbox 360 to check it with. Any ideas?

No, I get 2 beside my keyboard, as in 2 separate xbox controller icons are active. And when I move them left and right to choose a team, they both move at the same time. So for some reason, my computer is taking input from my one controller and giving it 2 separate outputs.
