GPU's voltage way down and cant change it

I have tried changing it in msi after burner, oc guru 2, CCC and i have also swapped out all the parts. So i'm guessing it's a hardware problem but i don't know what the problem is or how to fix it.

Can someone help please.

corsair cx-500. I know its enough watts, i have even tried a 600. my 12, 5 and 3.3 are reading perfectly. My gpu is a gigabyte r9 270x, i am getting 0.0001 minimum voltage.
