Case Fan Connection Preventing Boot

I just completed a new build and my computer will not boot when the 4-pin Chassis Fan is connected to the mobo. I get one led flash on the power button and that is all, nothing attempts to run. Unplugged everything runs fine including the 2 case fans, however they run non-stop because they are not attached to the mobo. I also checked for contact shorts and ran the setup outside of the case with the same result. Am I looking at a mobo issue or am I underpowered?

mobo: ASRock Micro ATX H81M-HDS
PSU: Corsair CX 500w
CPU: i7 4790k
Graphics: EVGA GeForce GTX 650 1024MB
SSD: 240 gb Kingston
Ram: 8gb Kingston Hyper X
 
Solution
The plug from the psu is NOT a fan plug. Its the power connector for a floppy drive. The only plugs used from the psu on a motherboard are the 20+4 and the 4+4. The rest of the power connectors go to components, not the motherboard itself. The motherboard then in turn powers the fans and other integrated stuff. The fans will connect either to the motherboard or the psu, preferably the motherboard. Hopefully you did not fry your motherboard by applying power from the psu in the back door.

Ok...I see the 3 pin one it is labeled from left to right....Fan Speed, +12v, GND
My plug from the PSU is....Red wire, Black wire, Black wire, Yellow wire
What way should I plug it in?
