Where to send rebate form of XFX r9 280x

I need help... I have been on the phone with newegg, XFX, and AMD. Where do I send this rebate in. I registered my r9 280x on xfxforce.com and I followed neweggs instructions on printing the papers out getting the UPC... etc. Now where do I mail that in?? There is no instruction?! I have an envelope with the papers in it ready to send, I applied for the rebate already online but I am pretty sure they need these papers... Help me please asap

did a fake one, at the end it said if you want to
Print Your Rebate Form Email Your Rebate Form

I registered got an email and it had a tracking number and all that but then why would they have me cut off my UPC from the box and print the rebate form?

if you have the tracking#, replace it on the link that I post, and your zip code too
