PC slow down, SMART log attached

So in the evening everything went well, was playing my game. The PC shut down properly, nothing bad happened. In the morning, the win 7 didn't load up, after leaving it load for like 5 minutes, i restarted, still nothing after. After like 2 hours i managed to load windows, but everything was waaay slower than before.

I reinstalled windows 7, no change..Then installed win 8. It worked very good for like 2 hours, then again..slow like a tortoise.

I installed HddScan for windows, and it gives me the following SMART report :

Except the bolded 3 lines, all had green light, those had yellow warning sign.

I installed Hard Disk Sentinel too, and it says my performance is 100% but health only 16% :

There are 439 bad sectors on the disk surface. The contents of these sectors were moved to the spare area.
The drive found 10 bad sectors during its self test.
There are 10 weak sectors found on the disk surface. They may be remapped any time in the later use of the disk.
328 errors occured during data transfer. This may indicate problem of the device or with data/power cables. It is recommended to examine and replace the cables if possible.
It is recommended to examine the log of the disk regularly. All new problems found will be logged there.

It is recommended to backup immediately to prevent data loss.

The notebook is an Asus U36SG, and it's 1 year old. Never had a hardware problem since now.
Any advice? Or something?
