Memory card disappeared withn 5 seconds in windows

I have a strontium 16gb micro SD.

I can't format my memory card in any devices.

I tried android, WP, symbion etc..

but i cannot format my card.

When i plugin my card using a card reader in windows its disappeared within 5 seconds

I am unable to format my card

No i am able to read all the data in memory hard.

I have 10gb data in that card.

When i insert my card in my galaxy grand 2 it says damaged sd card.

The problem is , card disappeared from windows within 5 seocnds
